The purpose of the Office of Student Conflict Resolutions is to promote, engage, and hold students accountable to the standards and core values of Oklahoma City Community College (OCCC). The Student Code of Conduct and associated processes provide students with the knowledge of their rights, responsibilities, and options of resources at OCCC. Furthermore, the Student Resolution Process offers optional avenues to students, faculty, and staff to address, report, and receive support, and/or dispute any incidents of misconduct that would hinder or interfere with the educational process or safety of the OCCC community.
Use the Student Conduct Incident Report form for:
- Student of Concern Reporting: This is a NON-EMERGENCY reporting form. This form is to be used if you are concerned for the well-being of an OCCC student. Concerns include, but are not limited to, food insecurities, housing issues, financial issues, etc.
- Student Conduct Incident Reporting: This form is to be used if you feel a student’s behavior does not align with the expectations outlined in the Student Community Guidelines. Such behaviors include but are not limited to, vandalism, disorderly conduct, drugs and alcohol. See the Student Community Guidelines for more information.
- CHAT Reporting: This form is to be used to address situations in which students, faculty, or staff are displaying behaviors that are concerning, disruptive, or threatening in nature and that potentially impede their own or other’s ability to function successfully or safely.
Use the Title IX Reporting Form for:
- Reporting Discrimination, Harassment, and Retaliation (including Sexual Harassment and Disabilities): The purpose of this form is to report instances of discrimination, harassment, and retaliation based on protected class/characteristics included but not limited to instances of Sexual Harassment (Title IX) and Disabilities to the Office of Institutional Equity.
- Requesting Title IX Parenting and Pregnancy Adjustment: The purpose of this form is to assist the College in determining what accommodations or adjustments are needed and can be granted to the student by the Title IX Coordinator.Â
- ADA Request for Reasonable Accommodations: The purpose of this form is for faculty and staff members to request reasonable accommodations under the American with Disabilities Act (ADA) form the College’s ADA Coordinator.
